(i)Submit the original and copy of the permit and the printed copy ofthe information in the QR code with the foreigner's work permit.

(ii)Submit the original and photocopy of class L B card of Shanghairesidence permit valid for more than 5 years (including 5 years)

（二）提交有效期5年以上（含5年）《上海市居住證》L類B卡原件及復印件線

提供下列之一與申請人身份和事由相符的證明材料

(i)Those who have won the "honorary citizen" or "Magnoliahonorary award" or "Magnolia Memorial Award" ofShanghai; The original and photocopy of relevant certificates and theletter of recommendation from the Foreign Affairs Office of themunicipal government shall be provided

（一）獲得上海市“榮譽市民”，或“白玉蘭榮譽獎”，或“白玉蘭紀念獎”的人員；應提供相關證書原件及復印件及市府外辦推薦函

(ii)The legal representative, general manager, deputy general manager andchief financial officer of high-tech enterprises, encouragedforeign-invested enterprises, foreign-invested advanced technologyenterprises or foreign-invested product export enterprises; Theoriginal and photocopy of the certificate of recognition of high-techenterprises, the letter of confirmation of foreign-funded projectsencouraged by the state, the certificate of confirmation offoreign-invested advanced technology enterprises or the certificateof foreign-invested products export enterprises shall be submitted

（二）高新技術企業、鼓勵類外商投資企業、外商投資先進技術企業或者外商投資產品出口企業的法定代表人、總經理、副總經理、財務總監；應提交《高新技術企業認定證書》、《國家鼓勵發展的外資項目確認書》、《外商投資先進技術企業確認證書》或《外商投資產品出口企業證書》原件及復印件

(iii)The legal representative, general manager, deputy general manager andchief financial officer of the multinational company's Shanghairegional headquarters (or headquarters type institution), R & Dcenter and investment company; The certificate of the regionalheadquarters of multinational companies, the reply on theestablishment of the R & D Center issued by the MunicipalCommission of Commerce or the original and photocopy of the businesslicense of the investment company shall be submitted

（三）跨國公司上海地區總部（或總部型機構）、研發中心、投資性公司的法定代表人，總經理、副總經理、財務總監；應提交《跨國公司地區總部證書》、市商務委出具的關于設立研發中心的批復或投資性公司營業執照原件及復印件經

提供下列之一與申請人身份和事由相符的證明材料

(iv)Foreign experts and scholars invited by domestic key institutions ofhigher learning, scientific research institutes and well-knownenterprises; The recommendation guarantee letter issued by theemployer shall be submitted

（四）國內重點高等院校、科研院所和知名企業邀請的外國專家學者；應提交聘雇單位出具的推薦擔保函件

(v)Foreign high-level management and professional and technicalpersonnel recognized by the competent department of talent and thecompetent department of science and technology innovation inShanghai; The confirmation letter issued by the relevant competentdepartment and the recommendation guarantee letter issued by theemployer shall be submitted

（六）國內重點發展領域、行業引進的創新創業團隊外籍成員；應提交上海科技創新職業清單所屬單位出具的推薦擔保公函、團隊負責人的有效身份證件及其出具的推薦擔保函件

(vii)Foreign members of the working team led by foreigners withsignificant and outstanding contributions and special needs of thecountry; Relevant supporting materials of foreigners (team leaders)with significant outstanding contributions and special needs of thecountry (such as award certificates, medals, employment certificates,etc.), valid passports and residence certificates of the teamleaders, originals and photocopies, and letters of recommendation andguarantee issued by the team leaders shall be submitted

（七）由有重大突出貢獻以及國家特別需要的外國人帶領的工作團隊外籍成員；應提交有重大突出貢獻以及國家特別需要的外國人（團隊帶領人）的相關證明材料（如獲得獎項證書、獎章、任職證明等），團隊帶領人的有效護照、居留證件原件及復印件，以及團隊帶領人出具的推薦擔保函件

(viii)Foreign high-end technical personnel with a term of five years; Theconfirmation letter of foreign high-end technical personnel issued byShanghai Science and Technology Commission shall be submitted

（八）任期為五年的外籍高端技術人才；應提交上海市科委出具的外籍高端技術人才確認函

(ix)The legal representative of an enterprise with a registered capitalof more than US $3 million General manager, deputy general managerand chief financial officer; The original and photocopy of thebusiness license or relevant registration certificate and the capitalverification report of the enterprise with a registered capital ofmore than US $3 million shall be submitted

（九）注冊資金在300萬美元以上企業的法定代表人.總經理、副總經理、財務總監；應提交注冊資金在300萬美元以上企業的營業執照或有關登記證明原件及復印件和驗資報告

(x)Apply for work residence certificate for more than 1 year (inclusive)in Shanghai for two consecutive times and there is no violation oflaws and regulations; The applicant shall apply for the work typeresidence permit in Shanghai for two consecutive times without anyviolation of laws and regulations, and submit the original andphotocopy of the foreigner's work permit and the printed copy of theinformation in the two-dimensional code

（十）在滬連續兩次申請辦理1年（含）以上工作類居留證件且無違法違規問題的；應在滬連續兩次申請辦理工作類居留證件、且無違法違規問題，并提交《外國人工作許可證》原件及復印件以及二維碼內信息打印件照

(xi)Personnel holding class a alien work permit with a validity of 5years, and personnel holding class L Shanghai residence permit with avalidity of more than 5 years (including 5 years) and B card; Theapplicant shall submit the original and photocopy of class aforeigner's work permit or class L Shanghai residence permit with avalidity of 5 years and provide one of the following supportingmaterials consistent with the applicant's identity and reasons

(xii)The legal representative, general manager, deputy general manager andchief financial officer of the headquarters of the privateenterprise; The certificate of recognition of private enterpriseheadquarters issued by the Municipal Commission of Commerce shall besubmitted

（十二）民營企業總部的法定代表人，總經理、副總經理、財務總監；應提交市商務委頒發的《民營企業總部認定證書》

(xiii)For the personnel working in the new port area of Shanghai Free TradeZone, the registered place and actual business place of the employingunit shall be in the new port area of Shanghai Free Trade Zone (forthe list of units, please refer to the administrative committee ofthe new port area of Shanghai Free Trade Zone)

（十三）在上海自貿區臨港新片區工作的人員，聘雇單位的注冊地和實際經營地均應在上海自貿區臨港新片區內（單位名單可詳詢上海自貿區臨港新片區管委會）

(xiv)Foreign Chinese working in Shanghai; Overseas Chinese need to submitChinese certificate for application. Please refer to the notice ofoverseas Chinese applying for certificate
